Create your own nameplate insert!

This is an OpenDocument file for making your own nameplate insert. You can open and edit it with Libre Office or Microsoft Word. (For example, you could put in a COTC logo!)

QR Codes and vCards

The image that looks like static is a QR code, which people can read with their smartphones. This one contains the data of a vCard, which is a sort of electronic business card. Scanning this code saves students the trouble of (mis)typing phone numbers or email addresses. You can of course delete it from the form if you don't like it!

To create your own, you can write the vcard yourself and use a tool such as qrencode to generate the image, or you can use a web service such as goqr.me. (With web services, note that the information you submit may be used for other purposes. This particular service claims that they do not retain or use any submitted information.)

Measurements

The form is made to fit the new office nameplates such as this one:

The measurements of the lower window are roughly:

  • 7.5 inches wide
  • 2.5 inches tall
  • 1 inch margins on the left and right sides
  • 1/32 inch margins on the top and bottom
